Insurance, Fees, and Payment Information

Fees, Payment, and Insurance

The time required for our highly trained and specialized physical therapists to render your evaluation and treatment is the basis for your fee. If we do participate with your insurance, we will file a claim on your behalf, but if Orthopedic Physical Therapy, Inc. does not participate with your insurance carrier, we will generate an insurance bill for your insurance company that you may submit. However, our fee is due in full at each visit. Failure to satisfy this requirement will void future appointments.

You must provide a current insurance card at time of service, and we suggest you be aware of your company's policy regarding physical therapy benefits, referrals, and preauthorization procedures to help avoid confusion. It is also important to know if you have 'out of network' benefits. Our experienced staff can assist you in clarifying these areas when you call to schedule your appointment.

For your convenience, Orthopedic Physical Therapy, Inc. offers several payment options, including MasterCard and Visa.
